DENTAL IMPLANTS IN SANTA CLARITA, NEWHALL, CA

Dental implants are a titanium artificial root replacement that can support a crown, bridge, or denture. They are surgically placed into the bone and allowed to heal for a period of 4-6 months. Once the healing process is complete, an abutment is attached to the implant, so that a prosthesis can be attached.

If there is insufficient bone available to place a dental implant, a bone grafting procedure has to be completed before implant placement. In some cases, the sinus will be too close and a sinus grafting procedure will be required. This process pushes the sinus cavity up from the area where the implant that will be placed.

With the new technologies today, almost all patients are good candidates for dental implants. Uncontrolled diabetes and smoking can decrease the success rate and the healing process.

What are a few implant related treatment options?

  • Single tooth replacement: A single tooth is replaced with a dental implant.
  • Multiple tooth replacement: Implants can support fixed bridges. This restorative option connects at least two or more implants together with teeth.
  • Complete arch replacement: 4-6 implants can be used to attach a full arch of teeth.
  • Over dentures: Implants can be used to help stabilize dentures that have minimal retention. These can help both the upper and lower denture.

What is the success rate of implants?

With the new technologies used today, implant success is above 98%. Routine dental checkups and x-rays are still required to ensure years of success.

Your Prosthodontist has the training to plan for the proper implant treatment for your individual needs. We begin by taking a CT scan to evaluate your jawbone. We can then plan for implant placement, in our office. Once the healing process is complete, the implant is ready to be restored. A mold is then made and sent to the lab for the restoration to be fabricated.

DENTAL BRIDGES IN SANTA CLARITA, NEWHALL, CA

A bridge is what you would expect it to be, Artificial teeth spanning an edentulous area. Bridges can either be removal or fixed.

A removal bridge commonly referred to as a partial has a metal framework to give it rigidity, and strength. Rest seats, which transfer some of the biting force to the remaining teeth, guiding planes that determine the path of placement of the partial, and clasps that hold the partial in place. While a lot of people do well with partials 40% of them live in drawers. Some of the complaints about partials are that they have a little movement, food gets under them, and there is more “stuff” in their mouth. Another issue with removable partials can be if there are no “back “ teeth to hold the end of the partial up it will sink in to the gums Andover time will not touch the teeth on the opposing arch the way it should. A fixed bridge is a restoration that is placed onto the teeth (called abutments) on either side of an edentulous area, with the missing teeth suspended between them. They can be made with a metal or zirconium base (see crowns), with porcelain placed on top of the base. Implants can also be placed and used as abutments.
Implants can also be placed in the edentulous areas so that a bridge is not necessary.

DENTAL CROWNS IN SANTA CLARITA, NEWHALL, CA

Crowns (some times called caps), are coverings over the biting surface of the tooth.
They can be partial coverings or total coverings. The important point is to cover the part of the tooth that makes contact with its opponent the opposite arch. Crowns are needed when the amount of tooth structure has been compromised. This can occur by decay, breakage or endodontic treatment (a root canal). When the size of the filling approached one half of the biting surface of the tooth the remaining walls
are weakened and there becomes a good chance the tooth will fracture off one of the cusps. Crowns can be made of different materials for different applications.

Porcelain fused to metal crowns, are sometimes referred to as pfm’s. A pfm is composed of a thin metal shell that covers the tooth. Porcelain is then added to the
metal shell, creating the tooth’s structure and anatomy. They have been the staple of most crowns and bridges for years. The draw back to pfm’s is that the metal they are made out of is grey. We would like the finished product to be translucent like a natural tooth. While it can be done it is hard to mimic the translucency of nature with the underlying color being grey. Never the less they have given years of good service.

We now have porcelain crowns that are strong in there own right, and do not need
the strength of the under lying metal coping. There are three main categories of these new porcelains.

The first is called e-max, and is a lithium disilicate. These are the crowns that can be made while you wait in a dental office often made with a digital impression. They can be very esthetic. They are usually bonded to the tooth, which increases their strength.

A similar crown can be made of leucitite-reinforced glass-ceramic it is called an empress crown. This too can be milled in the dental office with a digital impression. It is very esthetic, but not as strong as the e-max crowns. It too is bonded to the tooth.

The gorilla in the room is the zirconium oxides. It can be used by itself for a crown
But is not as esthetic as the above two. It can also be used as the metal coping with

A few Facts about dentures

The loss of teeth necessitates the need for dentures. Teeth are usually lost as the result of periodontal disease. Periodontal disease destroys the bone that supports the teeth in the jaw. When periodontal disease becomes severe enough the teeth become mobile and are eventually lost. What follows Is, that the remaining bone that originally supported the teeth is now used to support the dentures.
This brings up an interesting concept. The role of the dentist is to preserve bone not the teeth. If bone is maintained the teeth are maintained. If the teeth are lost, the only thing that we have to work with to support a denture is the bone.
A complicating fact is that the presence of the tooth in the bone keeps the bone healthy. Teeth are connected to the bone by small fibers and under normal function the pull and tug of the fibers keeps the bone healthy, when the teeth are gone the bone is no longer stimulated and starts to shrink this continues for the rest of your life. An interesting point is that if implants are put into the bone they will maintain islands of bone that can preserve the bone loss.

Dentures can be classified as complete replacing of all the teeth or partial, replacing some of the teeth.

Dentures are mechanical equivalents of natural teeth not a new set of natural teeth.
They work differently than naturally teeth and there is a learning curve to using them.

Some patients report a change of taste with complete upper dentures.

The amount of force one can bite with is reduced from around 200lbs. to about 20 lbs. resulting in a reduced chewing efficiency.

A suction seal can usually be obtained on the upper denture that helps maintain it in place. The lower denture just rests of the jaw and is easily displaced by the tongue and cheeks. Implants in the lower jaw are an option to prevent this complication.

Filling materials

In recent years there has be controversy about the different materials used to fill teeth. The original standard was gold, beginning with gold foils and gold inlays and onlays. The next material was silver amalgam. Which brought the cost of dentistry
Down for the average patient. The next generation of filling materials was fill resins, which were tooth, colored and had a great reception by the public. In recent times porcelain filings can be fabricated, and bonded into cavity preparation.

There has been so much discussion about the pros and cons of filling materials that the California state board of dentistry has got into the discussion. See The Dental Board of California dental materials fact sheet, at the Department of consumer affairs web site.

Amalgam filling material has been much maligned in recent years. It has been blamed for chronic debilitating diseases like Alzheimer’s, multiple sclerosis, and dementia to name a few. The fact remains that it has been proven to be safe and not responsible for these diseases. The problem with the material is that it has a mercury component. In the process of using this material some of it gets into the drains and gets into the ocean increasing the mercury in the environment, which is a reason to limit it’s use in my opinion. Another long term problem with amalgam fillings is that they can expand as they age and possible create fracture lines in teeth necessitating the need for a crown.

The next group of filling materials are the composite, resin reinforced, tooth colored filling materials. These materials have proven almost as long lasting as the older amalgam material but are more technique sensitive. Patients for the most part have

Glass ionomer are another resin that has a high concentration of fluoride. While they are not used in stress bearing area the can be used with patient who have dry mouth and a high rate of recurrent caries especially at the gum line.

Other than gold which I am not going to discuss because it has such little use now days the last material is porcelain. White fillings used to be called porcelain fillings, which was a misnomer because they were actually resins. With all the new digital technology porcelain restorations can be milled to fit a cavity preparation and bonded in place. They wear very well and can be very esthetic.

Orthodontics can also enhance your smile or be helpful in moving teeth to a position so that a veneer or crown can be placed to heighten your smile. Veneers are thin coverings of either plastic or porcelain that can change the shape or the color of an existing tooth. They usually necessitate the removal of 1 mm. of tooth structure, for their fabrication.
